Creating & Editing Consent Forms Using Rich Text Editor
Consent forms are used for events such as surgical procedures or for clients new to your practice. PetDesk Forms offers an entirely customizable form experience, or you can build on a standard consent form and edit it to your heart's content.
Creating Consent Forms using the Rich Text (RT) Editor

Then, select the green Create New button under the Consent Forms heading.
Start with creating a Form Name and then select the Form Type - the type selected will be the editor used to create the consent form. You can add and edit fields in either version.

Consent / Agreement Form Editor
Form Name: edit the form name in this open text box
Enable e-Signature**: adds an e-signature field to the bottom of the consent form; when enabled, the signature is required by default
Enable e-Signature - Customize: allows you to add a Signee Title (the title of the person who should be signing the form, like Client, Pet Parent) and/or an additional note or instructions above the signature line Note: The Forms system will add the word "Signature" after the custom "Signee Title," so you do not need to add this yourself
Make Signature Optional: If Enable e-Signature is checked, check this box so the signature is not required - useful if you need a live signature
Enable Provider e-Signature: useful for specialists if they also need to sign off on a consent form before a procedure
Enable Assistant e-Signature (Witness): allows for secondary signatures, in cases where someone needs to sign a consent form as a witness
Disable Form Header: removes any branding from this consent form
Access Control: gives you control over who shares or edits this consent form
Editor Controls: see standard controls for editing the rich text format for alignments, spacing, font, bullet points, inserting images, links, or tables, breaks, and more!
**E-signatures are viewable on the Consent Form when it is completed and returned:
Note: the client signature seen above is a typed version. The client may elect to sign with a free-form signature using a mouse, pen, stylus, or finger (depending on the device they are using). The typed version is also legal and will hold up in court based on the intent to sign and the signature attribution.
Click on the Certificate link on a submitted e-signature to view the Signature Certificate, which contains the signer's name, date, and time signed, and IP address.

Insert Field: a powerful feature that connects consent to your mapping and client information
Insert Form: allows you to build a form in the standard form builder, insert it into the rich text area, and then add additional fields or details
Placeholders: valuable if you intend to send forms from the PetDesk Forms Dashboard, but will not work as intended for a URL version of the form. This tag pulls in that information and enables personalization
Consent Form Editing and Creation
For the following example, we'll use a basicAVMA consent form as the basis.
Example in the Editor
Step 1: Paste the content into the editor
Step 2: Replace any lines that cannot be typed into with the Insert Field function
Step 3: Define the text box. In this example, the width of the text box is expanded from 100px to 400px, and this field is mapped to First Name.
Step 4 and Beyond: Repeat, adding fields as necessary for your consent form
