Consent forms are used for events such as surgical procedures or for clients new to your practice. PetDesk Forms offers an entirely customizable form experience, or you can build on a standard consent form and edit it to your heart's content.
Creating Consent Forms Using PDF Editor

Then, select the green Create New button under the Consent Forms heading.
Start with creating a Form Name and then select the Form Type - the type selected will be the editor used to create the consent form. You can add and edit fields in either version.

Upload Your PDF
1. Upload the PDF of your form.
2. Once uploaded, your PDF will appear on the right-hand side, where you can drag and drop additional fields over the existing PDF.
Fields & Form Name
Additional fields and settings are located on the left side of the editor.
Signature: can be dragged to any signature line
Initials: to require initials in the form
Textbox: the most common field you will drag into the form, used as a blank text field for open answers
Date Signed: specific to signature; do not mistake it as a date picker (that's usually applied to birth or 'gotcha' dates)
Checkbox: an easy way to insert a checkbox following questions with multiple choices, or yes/no
Staff Prefill: works well if you need to insert PIMS IDs for a patient OR add notes
Mapped Label: prefilled textbox that feeds off previously collected data
Drawing Canvas: where clients can draw on a diagram as needed. If you have an image of the patient and apply this field, they can draw on it with the mouse (e.g. owner draws on a dog diagram to describe its skin condition)
Actions
This menu lets you preview the work you've done on the form. Please note that you MUST click the green Save in the upper-right corner of the form to see the changes.
