Inserting PetDesk Forms URLs into Client Communications or Website

Say goodbye to manual data entry and time-consuming paperwork, and hello to a more efficient and professional clinic operation. Check out this video covering how to place the URL of a Form into PetDesk communications, including Two-Way Texting Templates, Appointment Reminder Messages, or Mass Messages and Mass Message Templates.
 

PetDesk recommends sending forms from the Forms Dashboard via email. If you prefer SMS, we recommend sending the form link via Two-Way Texting for better tracking and engagement. Steps for acquiring the URL to send via Two-Way are below and shown in this video. If you add your forms to your website, you must use the direct URL, not the embed code. 

  1. Select Form Templates in the upper right toolbar
  2. Select the desired consent form or Questionnaire 
  3. For Questionnaires, click on the three dots or Action menu in the toolbar above the form

  4. Select Get URL
    For Consent Forms, the Get URL link is already on the Consent Form editor menu: 
  5. Copy the Public Form URL

  6. Check I want this form to be accessed through my website or tablet if not already, to grab the direct URL
  7. Apply the URL to your desired PetDesk communication!

TIP! Use the direct URL to advertise the form directly on your website. Follow the below steps to create a "redirect" back to your website when clients finish filling out the form!

Redirect clients after they fill out a form on your website:

  1. Select Form Templates in the upper right toolbar
  2. Select your desired Questionnaire
  3. Click on the three dots or Action menu in the toolbar above the form
  4. Select Settings
  5. Enter your website URL in the When form is submitted, redirect client to field
