Additional Features for Consent Forms

Consent forms are used for events such as surgical procedures or for clients new to your practice. PetDesk Forms offers an entirely customizable form experience, or you can build on a standard consent form and edit it to your heart's content. 

This video and article will cover adding initials, dates, and checkboxes to consent forms using the Rich Text Editor.

Initials and Dates

In the case below, the mid-form signature is being replaced with a mandatory Initials field where it says Signed, and a Date Picker is being added to the Date field.

Once you complete your form, you'll see an interactive preview of your form where you can see the e-Signature portion. Note the arrow directing the user to Initial the document
