Creating & Editing Questionnaire Templates in Forms

The quick, easy-to-use PetDesk Forms questionnaire template editor creates a unique, specialized form for new and existing clients. Questionnaires are a different type of form that offers additional options not available in consent forms, but they can still be attached to a client's submitted consent forms!

New Questionnaire

From your PetDesk Forms Dashboard, click on Form Templates on the upper right corner.

Then, select the green Create New button under the Questionnaires heading.

Fill out the Name for your questionnaire form and select Continue.

On your first visit to the questionnaire editor, you will see a pop-up tour of the editor dashboard. If it's not your first time here, but you would like to preview this tutorial again, select "Show me how to use this page" on the right side of the questionnaire editor. 

 

Creating a Form

You can view the list of questions already added to your questionnaire on the left side window of the editor.

The right-hand side of the editor is where you can create, edit, duplicate, or delete questions. Use the green Add New Question button in the right corner of the editor toolbar to access this.

In this example, start by clicking Add New Question. Select the dropdown menu under Question Type and select Section Title / Note. This will allow you to state the form's purpose and expected goal for completing it.

For super-powered form abilities, utilize the + button on the left-hand side panel to pull up premade templates that combine several key questions. For example, the Add Demographic Question provides all the fields you may need for client contact information.

More powerful features include e-signatures (found under Question Type) to help create a legal aspect to the form to enforce rules such as cancellation policies or non-violence policies at your clinic.

Form Editor Navigation

In this navigation, you can return to the My Form Templates section by clicking the far left arrow. Additionally, you can Send to Client, Preview, Add Consent Forms, Save, and view custom settings for this specific form. Send to Client 

Clicking this button opens a simple window to send the client's form, requiring only their name and email address. You can include any additional instructions as well.

Preview

This will provide a preview of the form in its current saved state.

Consent Forms

You can attach your consent forms to your questionnaire intake forms and require a signature before the intake form is completed. Consent forms can be edited via PDF editor or Rich Text editor.

With the consent form successfully attached, the preview will display the following message requiring the client to sign the consent form:

⭘⭘⭘ (Form Settings)

The 3-dot menu, also known as the "meatball menu," is a home for many quick links and settings.

  • Questions Library: a portal to industry-specific questions, which is currently not optimized for the veterinary space
  • Print PDF: allows you to print a user-friendly PDF version of the form that can be manually filled out by clients
  • Duplicate: create a copy of the form in case you want to clone it and create alternative versions sharing similar questions or formatting
  • Get URL: One of the most useful features is grabbing the URL for your form so you can insert it into your PetDesk communication, such as Two-Way Texting or Mass Messages. The exact steps to do so can be found here
