Using Conditional Rules in Your PetDesk Forms Questionnaire

PetDesk Forms supports simple skip logic, allowing you to hide or show questions or consent forms based on a specific answer. You can set up skip logic on a multiple-choice question and indicate which questions/forms should be hidden or visible. This covers most scenarios we've seen in the forms we have converted. This will also work with radio button questions in the mixed control question type. 
 

Setting up Conditional Rules

First, create all the questions for your questionnaire, including conditional ones. Ensure the conditional questions appear after the questions that determine their visibility. Example:

 

Create a multiple-choice question: "Species" 
If the client selects "Dog", we can set logic to THEN hide additional questions for Cat, Equine, Exotic, Etc.

To create this example setup, follow the steps below:
Open the questionnaire rules by clicking the three horizontal dots (Forms Settings) > Rules.

 

Then click the Add New Rule button and set the parameters as shown below.

 

If we want to show only questions that pertain to species - Dog, we can hide the other species questions.

For #2 Equals, select 'Dog'. So far, this reads: IF the answer to the question of "species" equals Dog...

Set #3 Then to the action of hide question(s).
In #4, choose the associated questions that should be hidden.

So, put all together, this reads: If the answer to the question "Species" equals "Dog", then hide the questions "Your cat's breed" and "Your pet's breed".

Once complete, click Save, and the new rule will appear in the list of rules. It's possible to have multiple rules and conditional logic simultaneously.
