The quick, easy-to-use PetDesk Forms questionnaire template editor creates a unique, specialized form for new and existing clients. Questionnaires are a different type of form that offers additional options not available in consent forms, but they can still be attached to a client's submitted consent forms!
Below you'll find additional options for creating and editing questionnaire templates in PetDesk Forms.
To find this page:
- Go into the "My Templates" section of your PetDesk Forms Dashboard
- Select the indicated Questionnaire
- Select the "..." button and hit "Settings"
Form Settings
Settings

Form Name: a public-facing name for your form

Enable Interactive Mode: takes your survey and lays it out in a step-by-step wizard for online users

Show consent forms at the bottom instead of the top: moves any attached consent forms to the final step

Hide Progress Bar: for longer surveys, this helps prevent clients from abandoning the effort to complete the form

Hide Default Instructions: hides the system-generated instructions

Can be completed by the responsible party: allows a responsible 3rd party to fill out the form on behalf of the client

When the Form is submitted, redirect the client to: allows you to determine if you would like to send the user back to your practice's website after completing the form

Hide Print Button: If you do not want hand-filled paper forms, this will disable that button

Don't share this form with other practitioners in my organization: keeps forms separate from other practitioners; useful for multi-doctor practices

Prevent other practitioners from changing this form: locks form format
- Allow client to download form after submission: allows client to keep a local e-copy after completing
Invitation Email
Use this section only if this form requires a special email template.
You can set up a single email template for all your forms on the Settings page. Clicking Use a custom email template for this form will override the universal email template and allow you to create a custom one for this instance.
