Editing PetDesk Forms Settings

Easily create forms using templates. This allows your front staff to focus on delivering exceptional client experiences rather than on manual paperwork. Enjoy happier clients and better reviews, not to mention the convenience of them filling out forms online - time savings in the waiting room!

Form Header

Create a single form header that appears globally across your online questionnaire or consent forms.
Note: It is possible to disable or replace headers at the individual form level

PDF Header

Like the previous section, customize a unique header for the PDF print-out version of your forms, or opt to use the same header as the web version (select "Same as Form Header.")  


Emails

Customize the email template sent from your PetDesk Form Dashboard. 
Note: this does not impact the copy or templates from your appointment reminders, Health Service Reminders, or Mass Messages sent from your primary PetDesk client communications. These settings are not directly connected to a client's PetDesk communication preferences at this time.
 

SMS

Like email settings, create a standardized SMS when sending forms to your clients.
Note: texts sent from the PetDesk Forms Dashboard are not part of the standard PetDesk Two-Way Texting platform and are not connected to a client's PetDesk communication preferences at this time.
 
If you want to send a client a form via SMS, PetDesk recommends using the form's direct URL link and send it to the client through PetDesk's Two-Way Texting, for better tracking and client engagement.
 

Settings

Make specifications for your PDF and other settings. These settings are mainly up to personal preference!


 

Signature Preference

Allow your clients to either type or draw their signature or lock it down to drawing signatures only. Options include:
  • Prefer Drawing

  • Prefer Typing

  • Drawing Only

Date Field Type

Choose between a calendar-style date picker or a dropdown list. Options include:

  • Calendar Date Picker

  • Dropdown Lists

Public Form Login Method

Forms requiring a login may utilize an email address alone or an email or phone number. PetDesk suggests selecting "Require Email or Phone Number" for this setting, but options include:

  • Require Email

  • Request Email Or Phone Number

Default Form Delivery

Choose the standard delivery channel you want every time your form is sent. As discussed in the above SMS settings, PetDesk strongly recommends you choose your default delivery method as "Email," but options include:

  • Email
  • SMS

     

Email Sender

This is one of the MOST critical pieces of setting up your form! Determine the email address to send your forms - this will be the "from" address in the email. We suggest a shared email address that all your staff can access. If no email is selected, the "from" email will be noreply@petdesk.com.
 

Notifications

It's also essential to determine what internal notifications to set up. We encourage the defaults seen below - this way, you're notified of form responses in real-time as you work in the PetDesk Forms Dashboard:
