How to Send More Than One Form at Once

If you want to send more than one form to a client at once, you can create a package that includes all the forms that should be sent together.

A package comprises any combination of at least one form - a Questionnaire or Consent Form. Unlimited Consent Forms may be attached to one Questionnaire. There are two possible scenarios:

Package containing one Questionnaire + any number of Consent Forms:

1. Launch PetDesk Forms from the PetDesk Communications dashboard
2. Select Form Templates
   
3. Click on the desired Questionnaire
4. Click on the Consent Forms button located on the toolbar
   
5. Check the Consent Forms you want to attach to the Questionnaire (they will appear in the order selected)
6. Click Save

Package containing no Questionnaires + any number of Consent Forms

Use this workaround method if you want to send Consent Forms from the orange Send Form to Client button on the Forms Dashboard landing page. First, create a blank Questionnaire following these steps:

From your PetDesk Forms Dashboard, click on Form Templates on the upper right corner. 

Then, select the green Create New button under the Questionnaires heading. 

Fill out the Name for your questionnaire form and select Continue.

Next, follow the same steps to attach the Consent Forms to the blank Questionnaire:

1. Click on the Consent Forms button located on the toolbar
   
2. Check the forms you want to attach to the Intake Questionnaire (they will appear in the order selected)
3. Click Save
