You'll want to remove the provider card of your older location and follow the steps here.
Please note, you can always remove a provider card as needed. However, removing a card will not stop the reminders from a certain location. To stop those specific notifications, you'll need to contact the provider and have them mark you as inactive in their practice management software.
You're then encouraged to add your new provider card by following these quick steps when in the app.
